Appointments:

  • Please bring your cellphone, all communication will take place over the phone.
  • Your pet must be secure in a leash & collar.
  • Do not arrive more than 10 minutes early for your appointment.
  • Call the front desk at (909) 599 – 1508 from your vehicle when you arrive to check in.
  • Technician will call you when he/she is ready to bring your pet in to the hospital.
  • Once exam is completed the doctor will call you to review exam findings, answer your questions, and present treatment plan if needed.
  • Receptionist will contact you to collect payment over the phone.
  • Once your pet is ready to go a technician will contact you to meet them at the back door for discharge.
  • This is not a drop off exam, please do not leave the SDAH parking lot.
  • SDAH team members are required to exercise “Physical Distancing” please keep a 6 feet distance and refrain from holding a conversation with them face to face. All communication is over the phone, call us with all questions (909) 599 – 1508.

Vaccine clinic: Available by appointment only:

  • No exam fee will be charged because no exam will be provided.
  • Appointments available between 10am & 4pm Monday through Friday, Saturday: 10am to 3pm.

Surgical Procedures:

  • Consent forms & estimates will be emailed to you the day before. You must respond providing authorization & consent for the surgical procedure and/or treatment.

Medication & Food pick-ups: Please note, payment must be collected prior to pick up:

  • Please call 24 hrs. in advance to order prescription refills, food, flea & tick control.
  • Receptionist will call you to collect payment over the phone once your prescription has been authorized by the doctor.
  • The day you arrive to pick up please call the front desk from the parking lot, the receptionist will place your items on a chair located outside our front door for you to pick up.

NO WALK-IN SERVICES AVAILABLE:

  • No walk -ins will be accepted. This includes RX Refills, Prescription Food, and Flea & Tick Control. Client MUST call ahead of time and payment MUST be collected over the phone prior to pick up.

PAYMENTS:

  • All payments will be collected over the phone prior to pick up.
